1. PHOTO ID (Driver’s License/State ID)
2. PAYMENT (NO credit/debit cards)—-ONLY CASH OR CASHAPP PLEASE!!!
3. OHIO REASON CODE # (ORC) – given by your employer if applicable
4. CS Background check form  (will be provided at location to be filled or print and bring already filled out to the web check location.

WE ARE UNABLE TO DO HAZARDOUS WASTE AND CONCEALED WEAPONS AT OUR LOCATION.

PLEASE BE ADVISED THAT BCI REPLACED THEIR SYSTEM AND ARE STILL WORKING THROUGH SOME MIGRATION ISSUES THAT WILL CAUSE A DELAY IN PROCESSING BACKGROUND REPORTS.

CS IS NOT RESPONSIBLE FOR THE TIME FRAME IN WHICH REPORTS ARE BEING ISSUED BY THE OHIO ATTORNEY GENERAL’S OFFICE AND COULD TAKE 4-6 WEEKS TO RECEIVE.
